Some insurers ask for a formal letter of medical necessity before they'll cover breastfeeding equipment. That letter has to come from your OB, pediatrician, or lactation consultant, not from you. Bring or send them this template. It's meant for them to review, personalize, and sign. Please don't submit it under your own name.
